Welcome to Midwest Central CUSD 191!  Midwest Central is a consolidated school district comprised of students from Forest City, Green Valley, Manito, and Topeka.  We offer a traditional public school experience in a peaceful, rural setting.  Our high school and primary school are located in Manito and our middle school is located in Green Valley.  

The Midwest Central Educational family offers a supportive and rigorous environment for all students.  We encourage our students to take part in the variety of extra-curricular offerings whether it be a sport or club.  Our communities support our schools and offer cost effective living for families who are looking to raise their children in rural America.   

Midwest Central Announces Registration Dates for the 2018-19 School Year

Office of the Superintendent -- The Midwest Central public school system announced registration information for the 2018-19 school year.  To ensure the process is as easy as possible for parents and students, the location of registration is held in one place for all three buildings in the school district. 

 

Dates: Tuesday, August 7th from 7:00 a.m. – 7:00 p.m.

To expedite enrollment, parents are asked to bring documents listed below to registration and take note of the following information:

 

All students (district-wide):

  • New students are required to have proof of residency, a parent photo I.D. (at the Primary School only), a certified birth certificate from the county in which they were born, and immunization records.

High School students:

  • Incoming freshman should have updated immunizations along with a physical exam.
  • Basic book fees will be $80.00 (other fees will apply and vary by courses enrolled in).

     

    Middle School students:

  • 6th grade students are required to have physicals, a dental exam, and the appropriate immunizations.
  • A sports physical is required for all 7th and 8th grade Middle School students who plan to participate in sports.
  • Fees: Basic Book Fees - $85.00, Band - $7.00, PE Shirts and Shorts - $5.00 each, and 7th grade students and new 8th grade students to the district will be required to also pay a $7.00 fee for Constitution Books.

     

    Primary School students:

  • The Primary School Book Fees will be $65.00 for K-5th.
  • FREE Preschool is available either as a full or half day for 3, 4, & 5 year old children.
  • First time students and kindergarten students are required to have a physical, eye exam, and proper immunizations.
  • Students entering 2nd grade are required to show proof of a dental examination.

 

Immunizations required by the State of Illinois:

  • DTP - The month, day, and year of four or more immunizations, with the last booster received after the 4th birthday.
  • Polio - The month, day, and year of four or more immunizations, with the last booster received after the 4th birthday.
  • MMR - The first dose between twelve and fifteen months and the second dose no less than 4 weeks later.
  • Varicella (Chickenpox) - The IDPH is requiring any child entering kindergarten for the first time must show proof of receiving two doses of varicella vaccine or have documentation of prior disease. The healthcare provider must verify past infection.
  • Lead Assessment - Must be completed on physicals by health care provider for school enrollment for all children under age 7. Blood tests for lead screening may be done through your local health department.
  • Diabetes Screening - Actual blood tests are not required. However, the screening must be documented on the physical form by a healthcare provider.

 

We accept cash, check or credit card.

 

The Mason County Health Department will be at registration from 8:30 - 10:30 a.m. to host an immunization clinic.  Students must be accompanied by a parent/guardian to receive immunizations.
